Assurant Employee Benefits provides employees and retirees of the State of Tennessee a Prepaid Dental Plan, underwritten by Union Security Insurance Company.

We are committed to establishing and maintaining an excellent provider network, striving to deliver outstanding customer service, and providing our members with a dental plan that offers meaningful benefits.

How to Enroll

1. Complete the appropriate sections of the State of Tennessee enrollment/change application and return it to your agency’s benefits coordinator.  (If you need a copy of this application, please contact your agency’s benefits coordinator.)
2. Select a General Dentist for yourself and for every eligible member of your family.  Call Customer Service at 800.443.2995 or search here for a dentist. You can also nominate your current dentist if they are currently not part of our provider network.
3. Complete the Dentist Selection Form, being sure to include the 7-digit Dental Facility ID# for each Participating General Dentist you and your family select. Mail the completed Dentist Selection form to the address on the form.
